NAD therapy is one of the most searched, least understood corners of modern wellness. Marketing has run well ahead of the science, and most of what you'll read online either oversells the benefits or dismisses the whole field. The truth sits somewhere quieter, and more interesting, than either extreme.
This is a plain-language guide to what NAD actually is, what an NAD IV infusion does, what the current evidence supports, and what to expect if you book a session in Australia. It's written for people who want to make an informed decision, not a hopeful one.
What is NAD?
NAD, short for nicotinamide adenine dinucleotide, is a coenzyme found in every living cell in your body. It is not a drug, a vitamin, or a supplement in the conventional sense. It is one of the molecules your cells use to stay alive and functional, every second of every day.
NAD exists in two forms, NAD+ (the oxidised form) and NADH (the reduced form). The ratio between them is part of how your cells manage energy. When people talk about "NAD" in a wellness context, they almost always mean NAD+.

Why NAD+ declines with age
NAD+ levels fall steadily as we age. By the time most people reach their fifties, tissue NAD+ concentrations are estimated to be roughly half of what they were in early adulthood. Chronic stress, poor sleep, alcohol, and metabolic disease accelerate that decline further.
Lower NAD+ means less efficient mitochondrial function, slower DNA repair, and reduced sirtuin activity. This is part of why fatigue, slower recovery, and metabolic sluggishness creep in with age, even in people who eat well and train.

How NAD therapy works
Because NAD+ is required everywhere, raising it has become a target for clinicians and researchers. There are three main routes:
Oral precursors. Nicotinamide riboside (NR) and nicotinamide mononucleotide (NMN) are precursor molecules your body converts into NAD+. Human trials show they reliably raise NAD+ related biomarkers in the blood. Functional outcomes, things like measurable energy, metabolic markers, or physical performance, are more mixed and still being studied.
NAD+ IV infusion. Direct intravenous delivery of NAD+, typically 250 to 750 mg over 60 to 120 minutes. This bypasses the digestive tract entirely and delivers NAD+ into circulation. Subjective effects are commonly reported during and after a session. Rigorous randomised trials for IV NAD+ in healthy adults are still limited, a gap the field openly acknowledges.
Lifestyle inputs. Sleep, exercise (particularly Zone 2 and resistance training), fasting windows, and reducing alcohol all support NAD+ biology. These are the foundation. No infusion will outpace a lifestyle that depletes NAD+ faster than it can be replaced.
What does an NAD IV infusion actually do?
The honest answer: it raises circulating NAD+ to supraphysiologic levels for a short period. What that translates to clinically, in healthy adults, is still being mapped.
Patients commonly report:
- A clearer, more settled sense of mental focus in the days following a session
- Improved sleep quality
- Steadier energy across the day
- Faster recovery from training or travel
These are subjective effects, reported widely in clinical settings but not yet captured by large randomised trials. We think it's important to say that plainly. The mechanism is biologically real. The lived experience is widely reported. The randomised controlled trial evidence in healthy adults is still emerging.
If you see a clinic promising NAD IV as a treatment for a named disease, walk away. In Australia, that crosses both AHPRA and TGA lines. NAD IV is offered as a clinical wellness service, not a cure.
What the evidence does and doesn't support
A useful frame: separate mechanism, biomarkers, and clinical outcomes.
Mechanism. Well established. NAD+ is essential for mitochondrial function, DNA repair, and sirtuin activity. None of this is contested.
Biomarkers. Oral NR and NMN have moderate human trial data showing they raise NAD+ related biomarkers. IV NAD+ does the same, more dramatically and more briefly.
Clinical outcomes in healthy adults. This is where the gap sits. The 2025 Nature Aging consensus review explicitly called for larger, longer, better designed human trials before strong clinical claims can be made.
That is not the same as saying NAD therapy doesn't work. It means the field is early, and the responsible position is to use NAD therapy as one input within a broader healthspan strategy, with clinician oversight, rather than as a stand-alone fix.
What to expect from an NAD IV session
NAD infusions are not quick. They are typically run slowly, over 60 to 120 minutes, sometimes longer. The reason is tolerance. Pushed too fast, NAD+ can cause a band of mild but uncomfortable sensations: chest pressure, flushing, nausea, restlessness. Slowing the drip rate resolves them, almost without exception. A 2026 Frontiers in Aging pilot study on IV NAD+ tolerability documented this pattern in detail and is a useful reference for clinicians and curious patients alike.
A typical session looks like this:
Clinician review. Medical history, current medications, and goals are discussed before any infusion is booked. NAD IV is not appropriate for everyone, and a proper review filters that out.
Cannulation. A small IV line is placed, usually in the forearm.
The infusion. You'll be seated or reclined for the duration. Most people read, work, or rest. Sensations during the drip vary. Skilled clinicians adjust the rate to keep you comfortable.
Post-session. Most people feel normal within an hour. Reported effects (focus, sleep, energy) tend to show up over the following 24 to 72 hours.

- Pregnant or breastfeeding women

Safety and side effects
NAD IV therapy is generally well tolerated when delivered slowly and under clinical supervision. The most common during-infusion sensations are:
- Chest tightness or pressure
- Flushing
- Nausea or stomach cramping
- Headache
These are dose- and rate-dependent. They resolve when the infusion is slowed. They are not allergic reactions and they are not dangerous in a clinical setting. A well-run NAD session is paced to keep you comfortable from start to finish.
